Appointment

HERE'S WHAT TO EXPECT...

It is recommended that an appointment be made prior to visiting our facility. This allows us to set the appropriate amount of time needed to attend to each client's needs. If you must cancel your appointment, please let us know as soon as possible to allow time for other patients, if necessary.

For your first visit, please bring:

  1. Prescription from your doctor.
  2. Insurance Information (see below)
  • Private Insurance - insurance forms
  • H.M.O. - authorization from primary physician or medical group
  • Medicare - medicare card / secondary insurance card
  • Medicaid - current medicaid card spend-down information
  • Worker's Compensation - bring addresses and phone numbers of contact person to guarantee payment for device prescribed.

 

Our office staff will verify coverage for your specific device and inform you as to whether we will be able to bill your insurance directly. We are also a member of POINT Health Centers of America, which may be listed as a provider with your insurance carrier. You will also be notified of any balance due at the time of delivery of the device.

You may also need to bring one or more of the following:

  • Gym shorts for castings or fittings
  • Comfortable shoes to be used with your orthotic or prosthetic device
  • Any such device you have been using
  • Loose fitting garments and a white t-shirt for back brace fittings.
